Job application form template

What it is

A careers page for a made-up neighborhood bakery with three open jobs, each with its pay, hours, duties and what helps. Choosing one starts the application in five steps: contact details and whether the applicant may work in the US; the job, a week grid of mornings, afternoons and evenings they could work, a start date and hours; up to four past jobs, education and a link to a CV; why they want the job and two references; and a review of everything with an edit button on each part. Each step checks what it needs before moving on. The draft is saved in the browser as they type, so closing the tab loses nothing, and the application prints as a copy for the applicant. It asks no date of birth and no address beyond a town.

Who it's for

Small businesses, shops, cafés, clinics and charities hiring without an applicant tracking system, who want applications in one list instead of an inbox.

What you can change

You ask in plain words. These are the kind of changes people ask their AI for:

  • “Replace Fernhill Bakery with my dental practice, hiring a receptionist and a dental nurse.”
  • “Remove the delivery driver job and add a weekend dishwasher at $18 an hour.”
  • “Ask whether they hold a food handler's card, yes or no.”
  • “Add a question about their favorite thing on our menu.”
  • “Change the work authorization question for the UK.”

How to make it yours

  1. Remix it. A copy lands in your library as a site of your own, at its own link. If you have no account yet, you are asked to make one, and the copy is made when you come back.
  2. Change it in Claude or ChatGPT. Add the Solta connector once, then ask for the change in plain words, for example: “Replace Fernhill Bakery in my Job application form with my dental practice, hiring a receptionist and a dental nurse.” It reads the site's code, makes the change and saves it as a draft. The link keeps showing the old version until you publish.
  3. Publish. Ask it to publish, or publish from the site in your library. The new version goes live at the same link, so nobody needs a new one.
  4. Turn the form on. This template posts the job, the applicant's name, email, phone and town, their right to work, availability, start date, hours, past jobs, education, CV link, answers and references to /_solta/forms/application. In the site's Site settings → Forms, switch that form on; until you do, nothing is kept. Collecting answers is on every plan (how forms work).

Questions

Where do the applications go?

The page posts each one to /_solta/forms/application, at its own address. Turn that form on in Site settings → Forms and every application lands in the Answers tab, one row each, with a CSV button. Until you do, an application stays on the applicant's device and the page tells them so. Collecting answers is on every plan: Free keeps 100 answers a month for 30 days and emails you a daily digest; Pro and Team keep them as long as you choose.

Can applicants upload a CV?

No. Solta's forms take text, not files, so the form asks for a link to a CV the applicant has online instead, and it is optional. The past-jobs step covers what a CV would.

Does it follow hiring law where I am?

It is a sample, not legal advice. It leaves out questions employers are usually told not to ask, such as age beyond being 18 or older, but the right questions depend on where you hire; check them before you publish.

Or start from what you already have

Paste the file your AI made, or publish it from the chat, and get a link of its own.
